A few useful details.

Does it work beyond Google Forms?

Yes. Upload History captures standard file selections and drops across supported website upload controls. Compatibility depends on how a website handles files; cloud-document pickers and some custom uploaders may work differently. You can help by reporting a workflow that needs testing.

Can I recover a file after deleting the original?

Yes, if Upload History saved a copy while recording was enabled and that copy is still in your archive. You can download it independently of the original. It cannot recover files attached before you enabled recording.

How much can I save?

The default limit is 50 MiB per file and 500 MiB for the archive. You can adjust the archive budget up to 2,000 MiB. Files that exceed your limits keep a history record without a saved copy when space for the record is available.

Does anyone else get my files?

Upload History does not send your files or history to us. The archive stays in your current Chrome profile, so someone with access to that profile may be able to access it. Your normal uploads to websites continue as usual. Read the full privacy policy.

What happens if I uninstall the extension?

Uninstalling can permanently remove your archive and its encryption key. Download any copies you want to keep first, and keep separate backups of important files.

Is Upload History free?

Yes. The project is free and open source under the MIT license. You can inspect the code, try the local build, and contribute on GitHub.
